New Patients

Thank you for entrusting part of your medical care to Dallas Glaucoma Specialists. Our new patient scheduling process is outlined on this page. All appointment requests begin with completion of our New Patient Intake form (link below). If you have any questions, please call our office at 469-421-2100.

Step 1: Complete the New Patient Intake form

To help us prepare for your visit, please complete our online intake form. Completion of this form is required before a new patient appointment can be scheduled.

Step 2: Our office will contact you to schedule an appointment

After we receive your intake form, our staff will review the information, create your patient account, and then contact you to schedule the appointment.

Please allow up to 2 business days for our team to review your submission and reach out to you.

Please note: Appointment requests submitted by voicemail or phone without a completed intake form cannot be scheduled.

Step 3: Preparing for your visit

Please arrive 5–10 minutes prior to your scheduled appointment time and bring:

  1. Government-issued photo ID (driver’s license, passport, etc)
  2. Medical insurance card
  3. All the eye drops you are currently using, both prescription and over-the-counter
  4. A list of all the prescription medications you are currently taking

If you were referred to our practice by another provider, please ensure that your referring doctor has forwarded your medical records to our office prior to your appointment.

Additionally, please check with your insurance carrier to determine whether a referral from your primary care physician is required in order to be seen by our practice.
